Dementia Memory Support

When you or a loved one is diagnosed with dementia, it can be an overwhelming or even scary time of your life. At Coastal Care Partners, we are dedicated to providing both clients and their families with the guidance and support to navigate the complexities of dementia. Each of our care managers are trained to provide exceptional dementia memory care support, so they can effectively connect and communicate with clients, no matter your loved one’s stage of dementia.

Through our Aging Life Care Management program, we will manage your loved ones medications, attend doctors appointments, keep families updated on the latest advancements, and more. If you also enroll in our In-Home Care program, our care companions are trained in dementia care and are able to help seniors thrive at home by maximizing their capabilities.

Hospice Support

When it is time for hospice care, Coastal Care Partners will work with your Hospice provider to help guide both client and family. During this sensitive phase of life, Coastal Care Partners’ care managers provide support and guidance for both the client and the families. With our collection of hospice care support services and our experience in end-of-life care, our team is here to give peace of mind to your family, so you can make the most out of each day with your loved one.

During our hospice care support program, we will provide emotional support to clients and their families, create an individualized care plan, manage medications, and more. If you also enroll in our In-Home Care program, our care companions are trained in end-of-life care and are able to help families during this challenging time.
